NEW FAMILIES TO CSD17: If you have a student entering Pre-K, Kindergarten, or transferring into the District, please contact the district office at 815-467-4315.

EXISTING CSD17 FAMILIES: please login to TeacherEase to register for the new school year. Classroom assignments will be available to registered families beginning on Tuesday, August 6 through your TeacherEase account. Families that have not registered will not be able to access this information.

Physical Examinations

Illinois State Law requires students entering Preschool for the first time, Kindergarten, 6th grade, and/or when transferring from an out-of-state school to have their required physical exams and immunizations completed by the first day of school, or they will not be allowed to enter class. Appointment dates will not be accepted. The health history portion of the physical examination form must be completed and signed by the parent in order for the physical to be valid.

Physical examinations may be acquired through your primary medical provider. If you do not have a primary medical provider, you may contact your school nurse or the Will County Health Department at (815) 727-8670.

Vision Examinations
As of January 1, 2008, all Illinois children entering Kindergarten or upon first entry into an Illinois school beyond kindergarten (grades 1-12) are required to have an eye exam preformed by a licensed optometrist or medical doctor who performs eye exams.
Each child is required to present proof of an eye examination as specified by the Illinois Department of Public Health administrative rules, prior to October 15th of the school year. Exam forms are available by downloading Eye exam form, and if needed a waiver can be submitted by using the Eye waiver form.
